Name / Address Change
Name Change
- Login to the Nurse Portal.
- Select Manage Profile (on the left under your address).
- Select Demographics.
- Select Request Name Change.
- Type in Current Name.
- Select Upload.
- Select + Add File.
- Find & attach the official documents that show your previous name.
- Select file name, then OPEN.
- Wait for virus scan to complete, then click SAVE.
- Repeat to attach a file with the official documents that show your current name.
- Scroll to the bottom. Select Save and Return to Dashboard.
Note: You must provide a document to verify your previous name (i.e. birth certificate, social security card, marriage license, divorce decree, High school diploma) AND a document that verifies your current name (i.e. divorce decree, driver’s license, social security card, marriage license).
Example: Upload a copy of your marriage license AND a copy of your NEW Social Security Card/Driver's License.
Address Change
- Login to the Nurse Portal.
- Select Manage Profile (on the left under your address).
- Select Demographics.
- Scroll down to the Contact Information box.
- Select the circle next to the PSOR that matches your new address.
- Select +Change Address on the right.
- Type in your new residential address.
- (Mark the box to the right if your residential and mailing addresses are the same.)
- Select +Change Address on the right.
- Type in your new mailing address.
- Scroll to the bottom. Select Save and Return to Dashboard.
