Name / Address Change
Name Change
- Login to the Nurse Portal.
- Select “Manage Profile” (on the left under your address).
- Select “Demographics”.
- Select ”Request Name Change”.
- Type in Current Name.
- Select ”Upload”.
- Select ”+ Add File”.
- Find & attach the official documents that show your previous name.
- Select file name, then “OPEN”.
- Wait for virus scan to complete, then click ”SAVE”.
- Repeat to attach a file with the official documents that show your current name.
- Scroll to the bottom. Select ”Save and Return to Dashboard”.
Note: You must provide a document to verify your previous name (i.e. birth certificate, social security card, marriage license, divorce decree, High school diploma) AND a document that verifies your current name (i.e. divorce decree, driver’s license, social security card, marriage license).
Example: Upload a copy of your marriage license AND a copy of your NEW Social Security Card/Driver's License.
Address Change
- Login to the Nurse Portal.
- Select “Manage Profile” (on the left under your address).
- Select ”Demographics”.
- Scroll down to the Contact Information box.
- Select the circle next to the PSOR that matches your new address.
- Select “+Change Address” on the right.
- Type in your new residential address.
- (Mark the box to the right if your residential and mailing addresses are the same.)
- Select “+Change Address” on the right.
- Type in your new mailing address.
- Scroll to the bottom. Select ”Save and Return to Dashboard”.
