Name / Address Change

Name Change

  • Login to the Nurse Portal.
  • Select Manage Profile (on the left under your address).
  • Select Demographics.
  • Select Request Name Change.
  • Type in Current Name.
  • Select Upload.
  • Select + Add File.
  • Find & attach the official documents that show your previous name.
  • Select file name, then OPEN.
  • Wait for virus scan to complete, then click SAVE.
  • Repeat to attach a file with the official documents that show your current name.
  • Scroll to the bottom. Select Save and Return to Dashboard.

Note: You must provide a document to verify your previous name (i.e. birth certificate, social security card, marriage license, divorce decree, High school diploma) AND a document that verifies your current name (i.e. divorce decree, driver’s license, social security card, marriage license).

Example: Upload a copy of your marriage license AND a copy of your NEW Social Security Card/Driver's License.


Address Change

  • Login to the Nurse Portal.
  • Select Manage Profile (on the left under your address).
  • Select Demographics.
  • Scroll down to the Contact Information box.
  • Select the circle next to the PSOR that matches your new address.
  • Select +Change Address on the right.
  • Type in your new residential address.
  • (Mark the box to the right if your residential and mailing addresses are the same.)
  • Select +Change Address on the right.
  • Type in your new mailing address.
  • Scroll to the bottom. Select Save and Return to Dashboard.
