For Arizona RN/LPNs needing verification sent FROM Arizona to another State

  • Visit NURSYS.com, select “Verification for Endorsement” and follow the directions.
  • The NURSYS verification fee is $30.00 per verification sent. DO NOT submit payment to AZBN for this verification.

For Arizona RNs/LPNs needing verification sent FROM Arizona to another Country

  • Submit a $50.00 license verification payment through the AZBN's online payment system.
  • Send an email to [email protected] with a copy of your receipt, license number for verification, and/or supporting documentations.
    • You must include the name and address\email address of the recipient in order to complete the verification process.
    • Verifications can take approximately 3 weeks to be completed.

For Out-of-State RN/LPNs needing verification sent FROM Another State TO Arizona

  • You MUST apply for Arizona licensure by completing and submitting an application through the Arizona Nurse Portal.

Nursys Verification

  • If your original state of licensure participates in sending verifications through NURSYS, visit nursys.com, select “Verification for Endorsement” and follow the directions. List of participating states. 
  • The NURSYS verification fee is $30.00 per verification sent. DO NOT submit payment to AZBN for this verification.

Non-NURSYS Verification

  • If your original state of licensure does not currently participate in sending verifications through NURSYS (PA, CA-LVN), contact the state directly to have verification sent by email to [email protected] or by mail:

  • DO NOT submit payment to AZBN for this verification. Your original state of licensure may have charges associated with the verification request.

For Arizona APRNs, CNAs, or LNAs needing verification sent FROM Arizona to another State or Country

  • Send an email to [email protected] with a copy of your receipt, license or certificate number for verification, and/or supporting documentations.
  • You must include the name and address/email address of the recipient in order to complete the verification process.

To request verification from Arizona for the purpose of obtaining licensure in another state or country:
